1. Proof of Date of Birth and Place of Birth 🗓️

• School Leaving Certificate

• Birth Certificate

• Affidavit – Annexure-A (for illiterate applicants only)

2. Proof of Identification 🆔

• Any One from:

  • 🚗 Driving License

  • 🗳️ Election / Voter’s Card

  • 🆔 Aadhar Card

  • 🏦 Bank Passbook with Photo (duly stamped from a Nationalized Bank)

  • 🏷️ Ration Card with Photo ID

  • 📜 Any State / Central Government Issued Identity Card (with current address proof)

3. Proof of Residence 🏠

• Any One from:

  • ⚡ Electricity Bill

  • 🚰 Water Bill

  • 🏛️ Municipal Tax Bill

  • 📞 Telephone Bill

  • 🔥 Gas Connection Bill

  • 🏷️ Ration Card with Photo ID

  • 📃 Lease Agreement

Note: Documents must indicate at least 1 year of consecutive stay of the applicant prior to the date of lodging the passport application.

4. Proof of Education 🎓

• Std 10th Mark Sheet

• Graduation Certificate

5. Other Requirements (If Applicable) 💍

• Marriage Certificate

6. For Government/PSU Employees 🏢

• No Objection Certificate from Concerned Authority

7. For Dependants of Government/PSU Employees 👨‍👩‍👦

• Annexure – B from Relevant Authority/Department

8. For Minor Applicants 👶

• Birth Certificate

• School Bonafide Student Certificate (if applicable)

• Valid Passports of Both Parents

9. For Tatkal Passport ⏳

• Verification Certificate issued by Designated Officer (on letterhead, duly signed and stamped) along with Affidavit Annexure – I.

10. Other Documents 📄

• Additional relevant documents may be requested by the Assistant Passport Officer (APO) or Regional Passport Officer (RPO) at the time of submission.
